What to bring with you
For most patients, the admissions process begins at the Patient Registration Department. Princeton Community Hospital Association requires demographic and financial information, as well as general consent and insurance authorizations, for all patients. Upon your arrival to the Patient Registration Department, please remember to bring:
  • Picture ID 
  • All active insurance cards 
  • Physician orders
Patients under the age of 18 must be accompanied by a parent or legal guardian when registering for services.

From the Main entrance to the hospital
From the Main entrance, take an immediate left to the Patient Registration Department. This area is open 24 hours a day, seven days a week. Phone: (304) 487-7677

From the Parkview Center entrance of the hospital
From the Parkview Center entrance, take an immediate right to the Patient Registration Department. This area is open from 6:00 a.m. to 5:00 p.m. Monday through Friday. Phone: (304) 487-7620

Scheduling an appointment for outpatient tests

Pre-registration
A convenient way for scheduled patients to pre-register for outpatient services is through the pre-registration process, which enables patients to pre-register online or over the phone rather than coming to the hospital prior to a scheduled appointment.

Please remember to bring your insurance card and physician's order along with picture ID on the day of your appointment and stop by the registration department to complete the paperwork.
