What is Home Health Care?

The services of PCH Home Health are especially designed for the patient who is ready to leave the hospital, but is still in need of professional medical care. In some cases, patients may be referred to PCH Home Health by a primary care or emergency room physician as an alternative to hospitalization.

With home health services, patients can receive quality medical care at home while recuperating in familiar and comfortable surroundings.

The patient’s family can also benefit from these services, as they minimize the disruption to family life occurring when a loved one is ill. Our nursing staff can teach family members to administer some of the health care procedures themselves, enabling them to take an active role in their loved one’s care.

How to Receive Home Health Care Services

Anyone can request home health services by calling 304-487-2333 or 1-800-442-3270. Services must be ordered by the patient’s physician, and approved by the patient or the patient's custodian or guardian.

To ensure that our patients and families are receiving quality care and are satisfied with our services, we are dedicated to resolving problems and concerns promptly.

If you encounter a problem with PCH Home Health services, it is best to try to resolve the problem directly with our staff.
