The Case Management Department assesses, plans, implements, coordinates, monitors and evaluates the services provided to meet individual health care needs, while promoting quality, cost effective outcomes. Case management requires health care professionals to function collaboratively as a multidisciplinary, multiservice team. The underlying philosophy is that each discipline has something valuable to contribute, and by working together, we can improve the patients outcome and use our limited resources more efficiently. The department concentrates its efforts in five main areas: enhancing quality of care, shortening length of stay, controlling resource utilization, assuring continuity of care and reducing costs. |
